Take Control of Your Health helps people with chronic conditions and their caregivers, overcome daily challenges and maintain active, fulfilling lives. Often, we do not recognize health issues and diseases that are chronic conditions. These may include, but not be limited to: diabetes, high blood pressure, migraines, chronic back pain, arthritis or other consistent long term issues that affect our daily lives. Participants will learn skills that help them take a more active role in their health and health care. Audience: Anyone dealing with chronic (long term) health condition, caretakers or adults 60+.
This interactive course provides the tools needed to manage symptoms, work with health care professionals, set weekly goals for behavior change, use relaxation techniques and to exercise safely and easily. In addition to class sessions, participants are also given homework assignments to reinforce learning and help participants develop a personalized plan that addresses their own needs and concerns. As a result of the program, participants learn about healthier ways to live, gain confidence and are motivated to manage their health and feel more positive about their lives. Take Control of Your Health is a 6-session program with each weekly session lasting two hours.
