Help Us Reach Older Adults Who May Be Socially Isolated
Many older adults experience social isolation due to living alone, health challenges, transportation barriers, caregiving responsibilities, retirement, language differences, or the loss of family and friends. The Senior Wellness Program provides social connection, wellness programming, outreach, case management, and resource navigation to help older adults stay engaged and connected.
We welcome referrals from:
- Physicians and healthcare providers
- Hospitals and health systems
- Social workers and care managers
- Senior centers
- Housing authorities
- Faith-based organizations
- Community-based organizations
- Family caregivers and advocates
- Civic groups such as Rotary Clubs, Suburban Women's Clubs, etc.
Who Should Be Referred?
Consider referring older adults who:
- Live alone or have limited social support
- Report feelings of loneliness or isolation
- Have difficulty accessing community resources
- Are homebound or have mobility limitations
- Have experienced recent life changes such as retirement, bereavement, or caregiving responsibilities
- Would benefit from increased social engagement and support services
